  7. Neck Lift or Chin Liposuction: Which Is...

Chin liposuction is usually more suitable when a double chin is mainly caused by localized fat and the skin has good elasticity. A neck lift is recommended when there is sagging skin, visible muscle bands, or a significant loss of jawline definition. In some cases, the best result is achieved by combining both procedures.

Why does a double chin develop?

A double chin can be caused by fat accumulating beneath the chin, reduced skin elasticity, changes in the neck muscles, or the structure of the chin and jaw itself.

It does not occur only in people who are overweight. Even someone with a stable weight may have localized fat beneath the chin because of genetics or facial anatomy.

As we age, the skin may lose elasticity and the platysma muscle can become more visible. In this situation, removing fat alone may not be enough to create a smoother-looking neck.

For this reason, treatment for a double chin should be selected only after the surgeon has assessed the fat, skin, muscles, and facial structure.

What is chin liposuction?

Chin liposuction, also known as submental liposuction, is a surgical procedure that removes localized fat beneath the chin and along the upper part of the neck.

The surgeon makes small incisions, usually beneath the chin or in discreet areas nearby. A thin cannula is inserted through these incisions to remove fat and shape the neck contour.

The goal is not to remove as much fat as possible, but to create a more harmonious transition between the chin, jawline, and neck. Removing too much fat can cause contour irregularities or make loose skin more noticeable.

Liposuction does not remove excess skin and does not directly correct weakened neck muscles. The result depends on the skinโ€™s ability to adapt to the new contour.

Who is chin liposuction suitable for?

Chin liposuction may be suitable when the main concern is a limited deposit of fat beneath the chin.

The best candidates usually have relatively elastic skin, a stable neck contour, and little or no sagging skin. After the fat is removed, the skin must be able to adapt to the new shape.

The procedure may also be helpful for younger people with a hereditary double chin that does not improve sufficiently after weight loss.

When the skin is thin, very loose, or affected by significant weight loss, liposuction alone may not produce the desired result.

What is a neck lift?

A neck lift is a surgical procedure that treats signs of ageing and sagging in the neck and jawline area.

During the procedure, the surgeon may reposition the tissues, tighten the platysma muscle, remove or reshape fat, and excise excess skin.

The incisions for a traditional neck lift are usually placed around the ears and may extend towards the hairline. A small incision beneath the chin may also be required to correct the muscles or treat fat in that area.

A neck lift addresses more structures than liposuction. It is therefore a more extensive procedure, but it can provide a more complete improvement when the problem is not limited to fat alone.

Who is a neck lift suitable for?

A neck lift may be the more suitable option when a patient has sagging skin beneath the chin, multiple neck wrinkles, or a loss of jawline definition.

It can also treat the vertical bands that develop when the platysma muscle weakens or separates at the front of the neck.

The procedure may also be recommended when there is a combination of fat, excess skin, and weakening of the deeper tissues. In these cases, liposuction alone does not address all the causes of a double chin.

Candidates should be in good general health, have realistic expectations, and have no uncontrolled factors that could interfere with healing.

What is the main difference between them?

Chin liposuction mainly removes fat. A neck lift can correct fat, excess skin, and muscle weakness.

If the skin is firm and the double chin is caused by localized fat, liposuction may be sufficient.

If the skin is sagging or visible muscle bands are present, a neck lift may provide a more complete and controlled result.

Surgical literature emphasizes that submental liposuction is not a comprehensive solution for every patient because it does not address all the anatomical structures that can create fullness beneath the chin.

Can a neck lift be combined with liposuction?

Yes. A neck lift and chin liposuction are often combined when the patient has both excess fat and sagging skin.

In this case, liposuction helps remove and contour the fat, while the neck lift repositions the tissues and removes excess skin.

The surgeon may also tighten the platysma muscle if it creates bands or weakness at the front of the neck. This procedure is called platysmaplasty.

The combination is not necessary for every patient. The treatment plan is determined by the patientโ€™s anatomy, not simply by a desire for a greater change.

Is the procedure determined by age?

Age can affect skin elasticity, but it is not the only criterion.

A younger person may have sagging skin after significant weight loss, while an older person may retain relatively good elasticity.

The surgeon must assess how the skin responds, where the fat is located, whether the platysma muscle is weakened, and how well projected the chin structure is.

For this reason, it is not accurate to say that liposuction is only for younger patients and a neck lift only for older patients.

Could the problem be related to the chin structure?

Yes. A small or recessed chin can make the area beneath it appear fuller even when there is not a large amount of fat.

In this situation, liposuction alone may not create the desired balance in the profile. During the consultation, the surgeon may also assess the shape of the chin and jaw.

In selected cases, fat contouring may be combined with chin augmentation or repositioning. This is considered only when the skeletal structure genuinely affects the appearance of the profile.

How does recovery differ?

After chin liposuction, swelling, bruising, tenderness, and a feeling of tightness in the area are common. A compression garment may be recommended to support the tissues during the first stage of healing.

A neck lift involves a larger surgical area. Dressings may be applied after the procedure and, in some cases, a thin drainage tube may be placed to remove fluid that could collect beneath the skin.

After a neck lift, the head is usually kept elevated, and excessive bending or turning of the neck should be avoided. Instructions vary according to the technique and the patientโ€™s condition.

The final result should not be judged during the first few days. The contour continues to change as swelling decreases and the tissues settle.

Will there be scars?

Chin liposuction is performed through small incisions placed in areas where they are as inconspicuous as possible.

A neck lift requires longer incisions, usually around the ears and along the hairline. When the muscles or fat in the centre of the neck need to be treated, an additional incision may be made beneath the chin.

The incisions are planned along natural contours to make the scars as discreet as possible after healing. Their final visibility also depends on the skin, wound care, and the individual healing response.

How long do the results last?

Fat cells removed through liposuction do not return in the same way, but the remaining cells can enlarge if the patient gains a significant amount of weight.

Maintaining a stable weight helps preserve the achieved contour. However, liposuction does not stop ageing, and the skin may continue to change over time.

Neck lift results are also gradually affected by age, gravity, weight changes, and skin quality. The procedure can restore the contour to an earlier-looking state, but it does not stop the natural ageing process.

What are the risks?

Liposuction may be associated with swelling, changes in sensation, contour irregularities, fluid accumulation, infection, or the need for a later correction.

A neck lift may involve risks related to anaesthesia, bleeding, wound healing, asymmetry, changes in sensation, and, rarely, nerve injury.

Individual risks should be discussed during the consultation and before informed consent is signed. Choosing a qualified plastic surgeon and an appropriate surgical facility is an important part of safety.

How is the right procedure selected?

During the consultation, the surgeon should assess skin quality, fat distribution, muscle activity, and chin structure.

The patientโ€™s health, medications, previous procedures, tobacco use, and desired outcome should also be discussed.

Front-facing and profile photographs can assist with planning, but the final decision requires a physical examination.

A good treatment plan does not automatically choose the larger procedure. It selects the intervention that addresses the true cause of the double chin with the necessary level of correction.
